Appointment Check List

For all patients

1. Picture ID and social security number
2. Insurance card
3. Personal contact information (mailing address, home/work/cell phone number, e-mail address)
4. Emergency contact information (name, home/work/cell phone number, e-mail address)
5. Primary Medical doctor name and contact information
6. Nephrologist name and contact information
7. Cardiologist name and contact information
8. Medication list
9. Allergy list
10. A medical history (if you have one)
11. Echocardiogram, stress test and coronary cath results (f you have had them done)
12. Reports (with images or films, if available) from any relevant studies (venograms, fistulograms, thrombectomies and any operative dialysis access procedures)
13. Wear a short sleeve shirt, as usually we will do a ultrasound of your arm
14. If something happens at the last minute and it is uncertain whether you will be able to make it to the office, please call 248-355-1100 and leave a message with a contact number. Someone will get back to you as soon as possible.
